Make sure all your notations are documented in the patient’s chart. Then report your finding to the Administrator of the day care .

does she have a case or social worker ,you could talk to? is it adult day care-then you can call the ombudsman, how about calling the state/dhs?
does she have advance directives/no code?- you might want to find out her wishes before pursuing treatment.
can’t you call her doctor yourself?,I am so not familiar with the daycare setting and the nurses role in that setting, it seems odd to me-that you cannot contact her md directly.
did you ask your supervisor how to resolve this issue?

Can she shake her head yes or no? That might be what she could do to help.
