i need 4 interventions. i have one.
intervention:
assist patient while ambulating.
rationale: assisting patient decreases risk for falls
normal criteria: patient is able to ambulate without assistance
please help. 4/4 problem care plans due Monday!

Does not usually improve. This is one of the core jobs in nursing care.

2. Remove things from the room (not neccessary to the patient’s care) that can pose a tripping hazard, like suitcases on the floor or a chair that a visitor may have moved earlier and didnt replace.
#3. Provide hospital supplied non-slip socks, or have family bring the patient some house shoes. Do not allow the patient to ambulate in regular socks.
#4, Always assure that the nursing call button is within the patients reach and in functional order. If it is not, the patient will have little choice but to get out of bed and ambulate around without the benefit of a nurse’s assistance.
#5. Monitor the patient much more frequently after any administration of narcotics as he will be at an even greater risk for falls due to unsteadiness, and may not be in a clear enough frame of mind to remember to call for a nurse when ambulation help is needed.
